Your Care Hub is a Caboolture-based team supporting people across the Moreton Bay region. We believe good support is relational: it grows from knowing someone well and showing up reliably.
Our mission
To support people with disability and psychosocial disability to build independence, confidence and belonging — with supports shaped around their goals and everyday life.
Our story
Our founder Caroline Durham's journey began in 2016, volunteering as a family support worker with CYMHS while studying a Graduate Diploma of Psychological Science. Working in the Moreton Bay region, she could see a real need for supports in mental health and residential care.
Caroline went on to begin a Master of Social Work, completing a placement in a long-term residential mental health facility just as the NDIS was rolling out across Queensland — an experience that planted a seed. Her studies were interrupted by a breast cancer diagnosis, but that time only deepened her resolve to do meaningful work. In 2018 she began as a youth worker with Churches of Christ, a role she held for three and a half years. She loved building relationships with the young people she worked with and watching them overcome the challenges they faced.
This work is personal for Caroline. She has lived experience of mental health challenges and anorexia nervosa since the age of fourteen, and she continues to navigate body image and depression day to day. Far from holding her back, that journey shapes how she shows up — working in this space reminds her what it is all about, and it is what makes her approach so human.
Ready for a new challenge and keen to build something of her own, Caroline registered the company and took a leap, bringing family into the team. Disability was a new domain for her, but she has not looked back. Connecting participants with the right support workers and watching them work towards their goals has been deeply rewarding — over time, they become like family.
Caroline's faith in God drives her to serve the community and the people around her through this work. The vision for Your Care Hub is bigger than services alone — to see complete social and spiritual transformation in the Moreton Bay City.
Today Your Care Hub provides support work and community access, psychosocial recovery coaching and — from September 2026 — The Pulse, a gaming, connection and community hub in the heart of Caboolture. It is the same belief that started it all: people do best when support is consistent, respectful and connected to community.
